How to apply for enrollment in FACTS: The first step in enrolling your child in FACTS for the following school year (September 2008) is the form that follows. Please fill it out and return it, along with a copy of your child's birth certificate, to FACTS, 1023 Callowhill Street, Philadelphia, PA 19123. New students are chosen by lottery in the spring of the year before each school year. (NOTE: This year, the lottery took place on February 26, 2007. All children applying now will be placed on the waiting list). Priority for admissions go to residents of Philadelphia. Fill out a separate form (and make a copy of your child's birth certificate) for each child that you wish to enroll in our school. If one of your child(ren) is picked by the lottery to be enrolled in our school, their siblings will have priority in admission to our school. If your child is selected in a lottery, we will mail you an admission notice and additional forms that you must fill out in order to secure a space for your child.
